One of the facelift techniques that has recently become popular is the Extended Deep Plane Facelift. While all facelifts, when performed by a skilled plastic surgeon, can correct sagging midfaces, bags under the eyes, deep nasolabial folds (from the corners of the nose to the corners of the mouth), hanging jowls, a loose jawline, “turkey-gobble” neck, vertically tight cords on the neck (“platysmal bands”), and lax facial skin, the Extended Deep Plane Facelift effectively targets particularly hard-to-treat areas. These areas include the nasolabial folds, the marionette lines, the downturned corners of the mouth (“fish mouth”), and the jowls that cross over the jawline, causing the dreaded “W” effect that can be seen on profile.
Board-certified plastic surgeon Dr. Barry Weintraub often performs Deep Plane Facelifts or Extended Deep Plane Facelifts for patients whose issues reside in the lower-face and neck area. It is crucial to work with an experienced board-certified plastic surgeon, since operating on the deeper plane gives greater access to, and visualization of, the motor nerves of the face, so it is not a procedure for a novice or inexperienced surgeon. This approach also allows for easier access to the buccal-fat pads, the partial removal of which can provide welcome contour to the midface since, with age, the midface can become ill-defined due to the gravitational descent of musculature and tissues. (The key to natural-looking – and therefore successful – buccal-fat-pad removal lies in the importance of not removing all of the fat, which would produce an unnatural gauntness of the face.) “Subtlety,” says Dr. Weintraub, “is the focus of all of my surgical maneuvers in order to create results that look absolutely natural and unmanufactured, if you will.” Another area in which the Deep Place Facelift excels is that it allows Dr. Weintraub to release the mandibular-cutaneous ligament for a sharper jawline without the dreaded “W” effect that can be seen on profile. A further advantage of the Deep Plane Facelift is that, by providing tension to the deeper tissues of the face – like laying a fresh, firm foundation to facial structures – there is less pull on the surface of the skin, allowing for two benefits: 1)the skin gracefully sits atop this new substructure without tension, thereby preventing the “stretched” or “pulled” look; 2)it prevents undue tension on the incisions, which therefore do not widen over time.
Dr. Weintraub believes in achieving results that are natural and understated, always with a “less is more” approach and a keen eye towards natural attractiveness. You should look refreshed and rejuvenated after a facelift, never “fake” or “done.” All incisions are hidden so that any telltale signs of surgery are not apparent. The results of a Deep Plane Facelift can last longer than a more superficial facelift, and patients can expect to look refreshed in excess of fifteen years. As with his other facelifts, with meticulous and tedious focus Dr. Weintraub sews all incisions closed with exceedingly fine suture material while wearing magnification loupes for extreme precision.
Often Dr. Weintraub employs auxiliary procedures to enhance his delicate facelift work. These procedures can include: removal of fat from the jowls and neck, addition of fat to areas devoid of volume (such as the cheekbone region and areas around the mouth), as well as laser resurfacing to transform skin from burlap to silk.
