What are Fillers?

Juvederm Ultra, Juvederm Ultra Plus, Restylane, and Perlane are the most common hyaluronic-acid fillers. During the normal ageing process, the face undergoes shifts of subcutaneous fat and diminishing support by the deeper elements of the skin. Modern medicine has focused on this problem and in response, has developed filler materials with enough chemical integrity to correct the early signs of ageing.

The most popular and frequently used fillers are from the group of hyaluronic acids. Hyaluronic acid is a substance that occurs naturally in the human body and can be likened to the glue that holds your cells together. Juvederm and Restylane are very similar compounds most commonly used to address the nasolabial folds (the lines that go from the edges of the nose to the corners of the mouth), the marionette lines (the lines that go from the corners of the mouth down to the chin), the lips, and small contour irregularities of the face (acne scars, pits, divots, asymmetries).

The fillers are given by injection. Numbing cream is applied that Dr. Weintraub has custom made in California, comprising three different compounds, rather than the ubiquitous Emla that is found in most doctors’ offices. On rare occasion there can be a small bruise or some swelling, but in general, you can go back to work or your personal responsibilities immediately after the treatment.

The hyaluronic-acid products last on average 7-8 months. When looking in the mirror a few days after injection, you might even notice an increased improvement, as the product helps lay down the framework for stronger skin by making elastin and collagen.*

If one understands that wrinkles of the skin are caused by the slow weakening of a patient’s deeper dermal structure over time, then it is obvious that replenishing, or filling in, the base of wrinkles will result in repair. The goal of the hyaluronic-acid fillers (Restylane and Juvederm) is to act as the liquid building block, filling smoothly and naturally the unwanted wrinkle or fold.

Of the utmost importance in administering fillers is retaining the harmony and natural proportions of the face. Overzealous use and subsequent overfilling of natural and normal anatomic structures can produce unsightly results. It is therefore advisable to have these procedures done by board-certified plastic surgeons due to their thorough training and extensive knowledge of the deep anatomy of the body and its natural proportions.

Even though fillers are non-invasive, it is imperative that they be administered by a highly qualified practitioner because complications can result from improper application by inexperienced providers.   • Check for board certification by the American Board of Plastic Surgery (there are many other boards, some legitimate, some not, but there is only one American Board of Plastic Surgery, established in 1931)
  • Ensure that the physician attends a major hospital in your community, and is in good standing with that hospital
  • Confirm that he or she has a fine reputation amongst other medical professionals in your community
  • Speak to the surgeon’s patients who have undergone similar procedures

Remember, this is your treatment, and you are interviewing the doctor as much as he’s interviewing you. Do not hesitate to bring a list of questions with you on your consultation. And remember: a good surgeon is someone who not only has the skill to perform the treatment, but is someone who understands what your specific needs and desires are. Preparing For Fillers

Because fillers are non-invasive, there is no need to prepare anything in advance when planning a treatment. However, it is best to not take aspirin or aspirin-containing compounds for a week prior to injection.

After Treatment

Cold packs will be applied to the face or site of injection for a short period while in the treatment room. By the time you leave the office, you can return to your work or personal responsibilities with little or no down time.

During his surgical residency training, Dr. Weintraub regularly visited Mexico to donate his time operating on deformed children and adults who couldn’t afford proper healthcare. In less-than-optimal conditions, he learned how to perform surgery with the basics, and received great satisfaction in making profound differences in people’s lives by restoring their faces and bodies. Similarly, when he was Chief Resident at the New York Hospital – Cornell Medical Center, and later when he was Chief Resident at the Reconstructive Unit at Memorial Sloan-Kettering, he found that his compassion for patients, along with his skill and precision as a surgeon, could change lives and bring people joy they never thought possible.

Dr. Weintraub believes that surgery is never a race. The patient is always number one. He keeps in his wallet a piece of paper that is his motto in the operating room: “Always demand what is right, and never accept second best.” He likes to consider himself a “thinking surgeon,” and does not enter the operating room with a mechanical series of steps to execute robotically, but likes to remain alert and ready for whatever intraoperative challenges might spontaneously arise. Although many notable surgeons openly listen to music while performing surgery, Dr. Weintraub does not, preferring the mood of the operating room to remain serious and focused.

Over 60% of Dr. Weintraub’s practice is comprised of complex redos of surgeries performed by other offices. Such procedures always involve difficult issues, since there exist not only the limitations of a patient’s own anatomy, but also the limitations of another surgeon’s aesthetic, and the scar tissue that has developed as a result.
